For the OTT measurement, you add up all three. The circumference estimate will differ from the OTT estimate, but by how much depends on the shape of your pumpkin.
There are some drawing that better explain the measurements on this web site:

thanks for the link suggestion Bruiser!!!! Boy have I been measuring wrong, you must all think I am growing miniature pumpkins, all the measurements I have posted previously were only the circumference. The Big Max I pollinated on 8/5 is 11" circumference 8" Stem to Blossom 6" side to side Total 25" is that good for a Big Max at 6 days. Karen

Karen-
the OTT weight estimate doens't work to well for pumpkins other than AG's, Dills Atlantic Giants. AG's have very thick skin and weight more than a different varity that is the same size.

I am not concerned with the weight, as I am not entering any contests this year. I measure just to track the growth. For example the Big Max I pollinated on 8/7 (#5)has passed up the one pollinated on 8/5 (#3). On 8/12 #3 was 25 OTT, and #5 was 29 OTT. Today #3 was 26 OTT, and #5 was 38 OTT. Nine inches in two days for a Big Max....Karen
