Does your lab give you a buffer pH? This would be helpful to calculate the lime requirement. In general terms, I would apply about 200 lbs of Lime. Are you applying any manure this fall? Your K is low. If you are not applying any manure I would apply 13-15 lbs of sulfate of potash if you can find it. Humic acid this fall at the rate of 1-2 bages would also be good, with an additional 1-2 50 lb bages in the spring. Also 1-2 bages of Kelp meal in the spring.

The lab didn't list a buffer pH. but the lab recomended 6 t/ha of lime should be added. Ive already added 5000 lbs of mushroom compost along with some maple leaves.

Gypsum, lime(dolomitic), potash, maybe even some magnesium sulfate if lime doesnt add enough mg, or epsom salt.
