The bad news..every indoor grow cycle I have the problem in the picture. This has happened in a wide variety of grow media. I give my plants a systemic pesticide and fungicide. I have spent hours searching the net to determine the cause. For this round I am going to hypothesize that the plant is suffering from intumesence. LED grow lights state they have UV light but lower limit is about 380nm. This is barely getting into UV-A range. I have purchased a single bulb 2 foot t-5 24 watt fixture and placed a UV spectrum only bulb in it. It will emit the UV-B spectrum between 280-315 nm. I will be observing how this light affects the plant and make a determination if to add a uv only light to the rest of the plants.
